About the Department

The Chest and TB Clinics at Nyeri County Referral Hospital serve as a specialized unit for the prevention, diagnosis, treatment, and follow-up of tuberculosis and other chronic respiratory conditions. As a designated TB treatment centre under the National Tuberculosis, Leprosy, and Lung Disease Program (NTLD-P), the department provides comprehensive, evidence-based care aligned with national and WHO guidelines. We aim to control and reduce the burden of TB and related chest illnesses through accurate diagnostics, DOT (Directly Observed Therapy), patient education, and community collaboration.

  • TB Clinic Lead – Clinical Officer (Chest Specialist)

  • Chest Physician (Consultant – On Schedule)

  • TB Nurses – 2

  • Pharmaceutical Technologist – For TB drugs dispensing

  • Lab Liaison Officer – Coordinates sample referrals and results

  • Records Clerk – Ensures accurate TB notification and reporting

Patient Workflow & Process

  • Registration – Capture of patient details, TB number assignment for new cases

  • Triage & Screening – Symptom evaluation, risk profiling (HIV, diabetes, malnutrition)

  • Clinical Assessment – Physical examination and diagnostic workup request

  • Diagnostics – GeneXpert, sputum smear microscopy, chest X-ray, Mantoux test

  • Initiation of Treatment – Based on classification (DS-TB, DR-TB), weight bands, and regimen guidelines

  • DOT – Enrolment in Directly Observed Therapy Short Course (DOTS) with follow-up

  • Monitoring & Follow-up – Regular reviews, sputum conversion checks, side effect monitoring

  • Completion & Discharge – Clinical outcome documentation and reporting to NTLD-P

  • TB Diagnosis – Using GeneXpert, sputum AFB microscopy, chest X-ray, and clinical scoring

  • TB Treatment – First-line (DS-TB) and referral for DR-TB and MDR-TB cases

  • TB Preventive Therapy – Provision of TPT (e.g., Isoniazid Preventive Therapy for eligible patients)

  • TB-HIV Collaborative Care – Integration with CCC (HIV clinic) for co-managed patients

  • Chronic Chest Disease Management – Asthma, COPD, bronchiectasis, post-TB lung disease

  • Pediatric TB – Diagnosis and treatment of childhood TB including malnutrition support

Referral & Collaboration

  • Referral to County TB Coordinators – For MDR-TB and complicated TB cases

  • Integration with CCC – HIV co-infection screening, ARV initiation, and adherence support

  • Lab Services – Seamless coordination with hospital lab and NTLD-P reference labs

  • Linkages with Nutritionists – Nutritional support for undernourished TB patients

  • Community Health Volunteers – Assist in DOT monitoring and household contact tracing

Patient Education & Community Sensitization

  • TB Awareness – Education on transmission, prevention, and adherence

  • Infection Control – Advice on cough etiquette, isolation practices, and hygiene

  • Chronic Respiratory Care – Use of inhalers, lifestyle modifications, and breathing exercises

  • Public Outreach – Sensitization campaigns during TB Day and in high-burden communities
